The service fee includes a 30-minute follow up call reviewing your results. Most of these labs do not accept private or federal health insurance plans. To be covered by insurance, labs need to be ordered by PCP.
TO GET STARTED WITH ORDERING
Schedule an appointment 5 weeks out with the appointment being "Lab Test Consult." Testing takes 4-5 weeks to receive results (if your results come back sooner, we will let you know). Appointments are scheduled in your time zone when you provide your zip code. When your appointment is requested, you will be sent a link to create a username and password in the portal. This is how you will fill out the two required forms.
After you fill out the forms, you will be charged for the lab(s) and the associated lab fee. Your kit will then be shipped. The forms are sent to you in the client portal (CharmPHR) and you will have to log in to complete them electronically.
All tests have an additional charge for MN sales tax of 8%.
